Food Allergies

If your child has been diagnosed with a food allergy that may require the administration of emergency epinepherine please provide the following:

1. Allergy and Anaphylaxis Emergency Plan (updated each school year)
   • Must be completed and signed by healthcare provider
   • Signed by parent
2. Food Allergy Questionnaire 
3. The prescribed medication with a valid pharmacy label
   • Please check the expiration date of the medication
