              Middle School Health Requirements             

 

Immunizations:  New Jersey State Law and Cherry Hill School District require that children who are at least 10 years old or older and entering 7^th grade receive the meningococcal and Tdap (tetanus, diphtheria and pertussis) vaccines.

Physical examinations:  All middle school and high school students who wish to participate in any school sports activities are required to have a Sports Clearance Physical.  Starting this year, there is a new physical form for the physician and a clearance form to be submitted to school.  

I encourage you to submit immunization documentation and physical evaluations to me as soon as possible.  This information will be maintained in your child’s school health record, and given to the appropriate middle school nurse, when school health records are transferred in May.  This will make your child’s school health transition easier as they begin their middle school years.  All forms after June 1st should be sent directly to the middle school nurse.
